Pros and Cons of Cloud Computing Essay With the unprecedented growth and spread of information, there is no wonder why large Information Technology companies have been investing in the â€Å"cloud†. To expand, when referring to Cloud Computing, this means to access and store information not stored with in your computer whether it be public, private, or hybrid cloud computing. In technological terms, the server you are accessing is somewhere else and therefore, the information and tools you are using are â€Å"up in the clouds† but more so stored in another server space. However, whether you are paying for services through software, total usage, or free usage (hence the types of cloud services) Cloud Computing seems to be everywhere. Yet, it is important to recognize what is to gain by using cloud computing from big businesses to individual users and what are some risky moves when utilizing cloud storage over the Internet. Hence, the content of this paper will discuss the pros and cons of cloud computing and where cloud computing is headed in todays online society. Discussion: Moving forward, large companies use private cloud computing on the grounds that it is far more affordable to store and run programs in a server space designed for processing, saving and running information applications and so on. The biggest noticeable advantages in cloud computing have to be the ease of running programs with out installing any new applications (since the provider installs it for you in their cloud), and payment depends on membership, and which billing method you chose; for example, monitored payment or a flat rate (Mitchell). However, there are competing companies such as Google Apps Marketplace, Windows Azure, Amazon Web Services, Success Factors, and so on, that make cloud computing affordable on different user levels. These levels range from individual interest to larger company interests. For example, a company may need thousands of gigabytes worth of storage while I may need a few gigabytes of storage. So when paying for space I will be able to only buy what I need. Additionally, using the cloud allows a user greater mobility since an individual with an account can access the cloud from any computing device (laptop, desktop, tablet, or phone) that has available Internet access. More importantly, it’s easier for multiple users to access the same document and not overload the system and backs up data without someone having to worry about the location. Therefore, utilizing a third-party system server, Cloud Computing perks offer cheaper, reliable storage and retrieval of information. Figure [ 1 ] Although Cloud Computing sounds like a convenient service for businesses and users to take advantage of, there are downsides for paying or using cloud servers. Instances include, system shut downs for maintenance, access of data after canceling a membership, and more importantly privacy. Yet, a simple downside would be â€Å"What if someone uploads data into the cloud server and suddenly loses Internet connection?† The answer would be lost time and an incomplete data upload, not to mention the hassle of finding Internet connection again to access the cloud’s server only to attempt to upload the same work once more. On the other hand, looking back at the 2011 Google Docs incident, when an outage delayed user interaction for an hour starting in the UK and then reached the US (McCaney). Many organizations and people couldn’t access or exchange files via the cloud. So accidents happen, and although people lost out on time and information sharing, no files were reportedly lost. Even so, accidents including shutdown to outages, are rare natural occurrences, yet, inevitable for the current time. Not only are there outages to worry for also, according to Hamad Subani, he asserts that â€Å"Since most of the major Cloud Computing servers are operated by companies based in the United States, data you put on your Cloud is subject to American law.†   Which is also true since authorities only need to file a subpoena to get the information, yet, once information is shared warrants are needed and such (Subani). Hence, privacy is compromised, but to an extent. If someone is engaging in illegal file transfers on a cloud server, it already violates the consumer to provider contract; therefore a person is better off keeping their illegal activities to themselves. Other than that, cloud computing is relatively safe. | Cloud Computing Simplified| Pros| Cheap services, reliable, extra storage space, no worry about the server location| Cons| No privacy, and you must have internet access, maintenance. | After going through the pros and cons of cloud computing, it is necessary to analyze where such services are headed in the near to distant future. According to Brian Donaghy,   Ã¢â‚¬Å"Market Research Media says the cloud market will reach $270 billion in 2012 and Gartner predicts it will be over the $148 billion mark by 2014, much greater than Forrester’s forecast of over $118 billion for the same year.† Without a doubt, cloud computing will continue to rake in the profits since increasing needs for IT services are inflating the cloud’s development. In perspective, social networks (like YouTube or Skype) outsource their work to cloud services to help store user data. More so, it seems as if digital life is linked to the storage systems of cloud servers. Think about it, with out a back up storage in the cloud, there wouldn’t be a free YouTube, Tumbler, and blogging would essentially be news posts by big businesses. Cloud computing will continue to advance in ways that further accommodates all online users. About the only disadvantage of this are those with out Internet connection, the people left out of the Internet community. However, for the time being, cloud computing’s growth in the United States and American websites using cloud resources, prove to be helpful in the spreading of information from social to private perspectives of data usage. Conclusion: Hence, it is evident that cloud computing will be growing exponentially so long as the Information Technology sector continues to be in high demand. Therefore it is up to the consumers individual needs to decide if cloud computing would help them keep tabs with their data usage online or if using their own memory on their computer is a better decision. On the other end, companies should use Cloud Computing servers as a cheaper form of Information Technology management. Especially since investing in a mainframe computer would be far more costly than having some other company do all the work for you for a smaller fee. Therefore, cloud computing will definitely be a primary factor in the growth and facilitation of online data usage and continue to revolutionize modern technology. Sustainable Energy Sources Energy is life. Life on this planet depends upon a fixed amount of energy. The modern industrialized energy sources such as coal and petroleum were originally utilized for their seeming promiscuity and high energies yielded per unit volume. The world has now become painfully aware of how finite petroleum reserves are, not to mention the political complications associated with being dependent upon foreign countries for an energy supply. To add insult to injury, the limitedness of petroleum resources is not the most pressing problem, their polluting byproducts are. The enormous amounts of byproduct waste that finds its way into our environment is having effects which even the most renowned of experts cannot determine in magnitude. Speaking from an economic standpoint, petroleum will soon become more expensive to find and utilize than reasonable alternatives, raising it’s costs far above it’s benefits. Some could already make the point that the social and environmental cos ts never were weighed appropriately into the cost/benefit analysis of petroleum and that the benefits associated with petroleum combustion have rarely or never outweighed it’s costs. Anschluss – A Summary What was Anschluss? The Treaty of Versailles was created in 1919 designating all the new laws being enforced on Europe after WWI. Anschluss was one of these; it forbade Germany from making a political union with Austria. Due to this law Hitler had to find a way of making it seem as though Austria wanted the union. Hitler’s plan: He was boosted by his successes in 1936/1937 and now felt that Austria and Germany should become one German-speaking country. Austria felt the same due to its economic instability.The Nazi party was strong in Austria and Hitler encouraged his party to wreak havoc in Austria through demonstrations and riots. Hitler used this problem as an excuse to bully the Chancellor of Austria, Kurt Schuschnigg, into an agreement. Schuschnigg rebelled by turning the choice of a union to the public through a referendum. Hitler worried that he would not withstand this vote so he used force. His army was brought in as a persuasive element. C onsequently, 99. 75% of the Austrians voted for a union. Hitler had succeeded.This union had been allowed without the interference of either Britain or France. The British Chamberlain actually felt that the TOV was wrong to have disconnected Austria and Germany. Furthermore, Britain’s Lord Halifax had even suggested to Hitler that there would be no protest towards Anschluss on Britain’s part. Consequently, this meant that all of Austria’s wealth would add to Germany’s already expanding empire, through the form of weapons, soldiers, gold and iron. â€Å"The Nazi’s were creeping in like maggots into a dying animal. †

Hamlet is saying how his view of the world points out that all of his actions are wrong and a constant reminder of his inability to complete his revenge for his father’s murder by his uncle. Then points out that a man that only sleep and eat is nothing more than an anime but rather a man is meant explo it all of their senses and utilize for more meaningful pursuits than rather simply to survive.God did not give humans the ability to comprehend so they can ponder about the past and future and that those that are not used, the ability will slowly disappear. Also Hamlet reveals his thought of how he is alive when the deed he set out to accomplish is not completed even with all the desire and strength he has to accomplish the deed of avenging his father’s death. Another point that is revealed is Hamlet’s perspective on how an army goes to war over a worthless piece of land and make a mockery of the battles over that land.Then the soliloquy reaches its climax, Hamlet gives respect to the soldiers that are willing to sacrifice their life in battle for their kings honor and the honor of their land yet Hamlet has not taken action for his own personal honor regarding his father, his mother, and the state of Denmark where he is the known prince. At the end, Hamlet vows to thin k about nothing but his violent and bloody revenge for his father’s death against his uncle, Claudius and that any thoughts that is not violent is not worth thinking about.

Reconstruction of the United States after the Civil War essays The Emancipation Proclamation in 1863 freed African Americans in rebel states, and after the Civil War, the Thirteenth Amendment emancipated all U.S. slaves wherever they were. As a result of this free African Americans faced the problem of hostile whites around them preventing them to prosper. Despite the defeat of the Confederacy the nation was still unprepared to deal with the question of full citizenship for its newly freed black population. The Reconstruction was the time after the Civil War (1865-1877) when the United States was reorganizing the southern states and providing the means to accepting them back to the Union. Also to define how whites and blacks could live together, peacefully. These two major problems were placed on the weight of the soldiers of Andrew Johnson. Johnson became president after Abraham Lincoln. He was born in to a poor family, so he taught himself how to read and married a wife who taught him how to write. Before his presidency, he was the only senator from a confederate state to remain loyal to the Union. He hated wealthy slave owners. Before Abraham Lincolns assassination, he made it clear that he wanted a lenient Reconstruction plan. In December 1863, he announced his Proclamation of Amnesty which is also known as the Ten-Percent Plan. Lincoln's Reconstruction plan was designed to make an easy, peaceful reentry of the former Confederate states into the Union. Lincoln's policy was to forgive the South, rather than to punish them. Lincoln's Ten-Percent Plan allowed the former Confederate states to rejoin the Union when ten percent of the voters declared their loyalty to the Union. Lincoln's plan would let white southerners control the reconstruction process. Under this plan, Arkansas, Louisiana, Tennessee, and Virginia came back to the Union. Tempus is borrowed directly into English only in the phrase â€Å"tempus fugit,† an abridgement of a quotation from the Roman philosopher Virgil; it translates to â€Å"Time flies,† commenting on ephemerality or expressing impatience. Tempo comes directly from Italian and refers in English to the speed at which music is performed or to the rate of any rhythmic activity. Temporary means â€Å"lasting only for a time† and has developed into a noun referring to someone hired temporarily; this usage is often abbreviated to temp. Contemporary, meaning â€Å"characteristic of the present† or â€Å"modern,† literally means â€Å"with time.† Extempore (literally, â€Å"out of time† and pronounced â€Å"extempory†), formally means â€Å"without preparation or time to prepare† but has the additional connotation of â€Å"spontaneously.† It serves primarily as an adverb but can also be used as an adjective or a noun, although extemporaneous is the preferable adjectival form. To temporize, meanwhile, is literally to conform to the time by adopting to an established opinion, or to draw out discussion or negotiation to gain time, while temporal refers to that which pertains to time, not space, to measurable time as opposed to eternity, or to secular rather than sacred matters, or pertains to a specific time or to the sequence of time. A contretemps- the word is borrowed from the French term contre-temps (literally, â€Å"against time†)- originally described an error in fencing, from the notion of poor timing, and now refers to an argument or an embarrassment. Tense, referring to the form of a verb showing time, comes from the Old French word tens, which derives from tempus. (The sense of tense that means â€Å"strained† or â€Å"rigid† is unrelated; it is akin to thin.) The noun temper, meaning â€Å"composure,† â€Å"disposition,† or â€Å"passion,† as well as â€Å"courage† or â€Å"tone† (as in â€Å"the temper of the times†), or pertaining to the state of a substance, such as the feel of leather or the resilience of steel- and the verb temper, meaning â€Å"moderate,† or â€Å"attune† or â€Å"toughen†- is derived from the Latin verb temperare, meaning â€Å"moderate,† which is likely related to the original sense of tempus, which may be â€Å"stretch.† The Latin verb is also the source of temperature, which pertains both literally to relative heat or cold or figuratively to mood or emotional heat. (To say that someone is â€Å"running a temperature† means that the person’s body heat is abnormally warm.) The sense of temple that pertains to the area to either side of the forehead derives from the â€Å"stretch† sense of tempus, but the meaning â€Å"sacred building† is unrelated. Tempest, meaning â€Å"storm,† is from the â€Å"season† sense of the Latin term, as is the adjective tempestuous, which describes not only a literal storm but also turbulent behavior or a stormy relationship. America had finally invented the A-bomb, now they had this Russia had no importance to the United States whatsoever. But at the same time The U S were had their own suspicions of Russia as well. After the Second World War all the armies had removed their troops from Eastern Europe, but Russia still kept theirs there. The U S knew Stalin was up to something, and he was! Stalin took over all of the countries surrounding the Soviet Union. America was not happy about this at all. But Stalin promised that he would hold free elections in these countries and to not force communism upon them. He lied! He knew that the only way the west could stop him from creating the countries in to communists was to go to war and he knew they would not want to do that because of the consequences they had suffered in the War. America was not ready for another war she was financially crippled. With all the surrounding countries under his rule Stalin knew that if countries were to invade Russia they would have to go through these first. This created a barrier; it bought him time before anyone could reach Moscow to attack. Churchill described this blockade as the Iron Curtain. It separated Russia from the rest of the world. The U S decided they needed to take action, so they devised up a plan of Containment. This was a policy that meant that they would help any country to defend themselves if they were threatened by communism. America and the West were trying to make sure that communism did not spread. Then came the Truman Doctrine. Turkey and Greece were having a lot of problems with the Russians. They had to defend themselves against Russia and its communist regimes. Britain was aiding them with weapons and supplies but an economically crippling winter in 1946 meant that they could not afford to help no longer. So the U S came in to help. The Truman Doctrine that was made was a policy of Americas Commitment against communism. Any country facing a war against Communism would be helped by the U S. Then came another plan by the U S, this one was called the Marshall Plan. A man called George Marshall who was the Secretary of State for the U S at the time of 1947 devised it. George Marshall pledged that the U S government would give Billions of dollars to the poorer countries in Europe. On the outside this was looked upon as a very unselfish and kind gesture. But if looked upon closely you could see that it was just part of a scam. It was in Americas greatest interests to give Europe the money. The reason why? To stop communism! It did this because all the poorer countries were heavily stricken with poverty and hunger, the people were extremely dejected and needed something to look up to, more like a light at the end of the tunnel. To them with the help of Stalins propaganda schemes Communism looked like a way out of poverty. With the U S giving them money it made them happier, they had no more problems, thus giving America the edge on containing Communism away from vulnerable Countries. In 1962 a major development had been revealed. Russia was sending over nuclear missiles to Cuba. This meant that America was now under threat from nuclear attacks. But America quickly intervened they told Russia that if they were to send any more shipments over that they would take immediate action and destroy the vessels. Russia was not expecting that reply they assumed no action would take place. America set up blockades and arranged air strikes over the ocean; Khrushchev decided to deploy their ships back to Russia before another conflict took place. After seeing and hearing of these events taking place it must have been extremely frightening for the people in the world at this time. There was a huge threat of a nuclear war, which could have drastically destroyed the world completely. Each countries nerve was tested to the limit, but luckily no one decided to retaliate in a major way. With the U S inventing new plans and technology the west made it extremely hard for Russia and Stalin to take over the world and make it communist. The Cold war changed the world forever, no longer would wars be fought in trenches and battlefields. They would be fought with words, and with just one switch of a button the whole world could be destroyed.

Nursing Processes for Emesis Management Nausea and vomiting are common complications of multiple conditions, procedures, therapies, and events such as motion sickness, pregnancy, anesthesia (general, regional, or local) or radio/chemotherapy. Symptoms can be debilitating for many patients, and in the case of post-operative nausea and vomiting (PONV) physical damage may result, such as rupture of sutures, stitches, and esophageal tissue, and metabolic problems, such as electrolyte imbalances and dehydration (Golembiewski et al, 2005; Gan 2006). In severe cases of PONV, although rare, aspiration of gastric contents may occur, resulting in pulmonary sequelae, such as pneumonia or pneumothorax (Scuderi and Conlay 2003; Bremner and Kumar 1993). Thus effective treatment of PONV, possibly through multimodal antiemetic prophylaxis, is an important are of research (Skledar et al. 2007). This essay will consider two commonly used, well-recognized antiemetic treatments namely cyclizine and prochlorperazine. Both represent very old drug therapies, with cyclizine having been launched as an antiemetic in 1953, and prochlorperazine as an antipsychotic in 1957 (Broccatelli, 2010), its use as an effective antiemetic emerging soon thereafter (Finn et al, 2005). These drugs are commonly used on most wards in my practice setting and therefore it is vital for nursing staff to understand their respective pharmacodynamic (PD) and pharmacokinetic (PK) profiles. Prior to prescribing it is also important that the nurse have relevant knowledge regarding how these drugs work, how their PD and PK properties are altered by disease processes such as kidney/liver failure and whether there are any relevant contraindications or precautions. Additionally, the potential for drug-drug interactions and the dose appropriate for the patients age and weight should be ascertained if beneficial pati ent orientated outcomes are to be achieved. These issues will be comprehensively discussed within this essay. Pharmacology of emesis There are a plethora of drugs on the market to treat emesis, however, deciding upon an appropriate and effective treatment for patients requires the cause of the underlying nausea and vomiting to be ascertained. This is because the symptoms can manifest as a result of a number of underlying pharmacological processes, as will now be described. Vomiting is a complex reflex action controlled by the vomiting centre (VC) in the medulla region of the brain, an important part of which is the chemotrigger zone (CTZ); stimulation of this in turn leads to VC stimulation which ultimately leads to vomiting (Goodman Gilman, 1996). Neurotransmitter mediated stimulation of the VC can arise from both peripheral and central impulses (Shanbhag, 2008). Thus gastrointestinal irritation, motion sickness and vestibular neuritis all manifest in nausea and vomiting as a result of neurotransmitter release. The three main neurotransmitters involved in the control of vomiting are acetylcholine (ACh; via muscarinic-receptors), dopamine (via dopaminergic receptors), histamine (via H-1 receptors), and serotonin (via 5-HT3 receptors) (Shanbhag, 2008). Inhibition or antagonism of these receptors achieves emetic control. The VC has neurons which are rich in muscarinic cholinergic and histamine containing synapses and is directly stimulated by the vestibular input (e.g. through motion sickness), whilst dopamine and serotonin release are involved in the visceral stimuli pathway (e.g. through chemotherapy treatment) and also in the CTZ stimulation pathway as shown in Figure 1. Thus drug classifications of anti-emetics arise on the basis of which of the three pathways that they target (Flake et al., 2004). Selective serotonin receptor antagonists and antidopaminergics target the visceral stimuli and the CTZ, whilst the antihistamines and anticholinergics target the vestibular input pathway (Hornby , 2001; Flake et al., 2004). Etiology of Nausea and Vomiting Cyclizines anti-emetic effects are not fully understood but it is thought that it works by blocking the transmission of information from the labyrinthine apparatus in the inner ear (i.e. the vestibular pathway) to the VC (Goodman and Gillman, 1996). Cyclizine may also target the CTZ and it thought to exhibit some ACh muscarinic receptor blockade which probably contribute to the antiemetic potential thus operating at several pathophysiological levels. However, a side effect of ACh blockade is sedation in some individuals along with the potential for certain deliriant and hallucinogenic effects, probably responsible for cyclizines abuse potential (Bailey and Davies, 2008). Cyclizine produces its antiemetic effect within two hours and it lasts approximately four hours (emc, n.d.). The exact mechanism of prochlorperazines antiemetic action is also unclear, but the drug is thought to inhibit apomorphine induced vomiting by blocking dopamine D2 receptors centrally in the CTZ and possibly peripherally through dopaminergic receptors in the intestine (Perwitasari, 2011).   However, it also has some potential to block anticholinergic and alpha-adrenergic receptors, and therefore can also result in sedation along with muscle relaxation, and orthostatic hypotension (Kelly, 2000). Following intramuscular administration prochlorperazine has an onset of action within ten to twenty minutes and a duration of action of three to four hours (globalrph, n.d.). Indications and dosage form Cyclizine is indicated for the control of postoperative and drug-induced vomiting and in motion sickness (BNF, 2012; emc, n.d.). It is given by mouth at a dose of 50mg tablets up to three times a day or parenterally as a 50mg in 1ml solution intramuscular (im) or intravenous (iv) injection again at a frequency of up to 3 times a day (Reynolds, 1993). The recommended dose in children aged 6-12 years is lower: 25 mg up to 3 times daily. For motion sickness, it is recommended that tablets be taken 1-2 hours before departure. Cyclizine can also be given for vertigo and, morning sickness in pregnancy, and to combat  opioid nausea. It is also prescribed for radiation sickness (medsafe, n.d.) and PONV (Cholwill et al., 1999), indeed it is given iv before the induction of general anaesthesia at half the recommended dose, to increase the lower oesophageal sphincter tone thus reducing the hazard of regurgitation and aspiration of gastric contents (medsafe, n.d.). Although prochlorperazine is classified as an antipsychotic, its principal use nowadays is in the treatment of severe nausea and vomiting of various causes including, PONV, vertigo and motion sickness (BNF, 2012). It has several dosage forms: tablet (5mg: one or two tablets 3-4 times daily), syrup (5mg in 5ml: 5-10 ml 3-4 times daily), suppositories (25mg twice daily), dissolvable tablet (buccal tablet 3mg: one or two tablets twice a day in adults and children aged 12 years and over), im injection (12.5mg in 1ml; 5-10mg repeated every 3-4 hours with a maximum daily dose of 40mg) and iv injection (2.5 -10 mg by slow IV injection or infusion  with a maximum daily dose of 40mg). The oral (and buccal) route is the only method of administration recommended for children, and it is not recommended in children younger than  12 years (BNF, 2012). The different dosage form of prochlorperazine provides the nurse with flexibility for example the elderly and children may prefer the syrup or b uccal tablet, or in dysphagia suppositories or intra-muscular injections could be more appropriate. Cyclizine and prochlorperazine are both considered first line treatments for nausea secondary to vertigo and motion sickness (Quigley, 2001) and are first line treatments in many hospitals in PONV (NHS, Salisbury; NHS Plymouth). A review by Matchar, et al. (2003) has suggested that oral prochloperazine may also be used as an adjunct in the treatment of nausea associated with migraine (Matchar et al, n.d.). No randomized controlled trial has been found which formally compares efficacy of cyclizine and prochlorperazine, however, two studies comparing cyclizine with perphenazine in ameliorating drug-induced emesis, have shown the former to have comparable antiemetic efficacy to this related phenothiazine drug (Dundee et al., 1975; Chestnutt and Dundee, 1986). These studies are featured in a Cochrane report (Stevenson, 2006) which investigates drugs for preventing PONV and highlights eight drugs which reduce PONV by a similar amount in this patient group, cyclizine being one. The report concluded, therefore, that the most important question to answer when treating emesis is What are the types and risks of side effects experienced by patients exposed to these antiemetics? Thus safe and effective prescribing requires the nurse to identify patient variables or comorbidities relevant to the drugs side effects, for example heart failure patients should not be prescribed cyclizine and individuals susceptible to visual disturbances should avoid prochlorperazine as per the drugs contraindications. It is noteworthy that both drugs may be prescribed in the later stages of pregnancy if considered appropriate by a doctor (Schaefer, 2007; CKS, n.d.).  [1]   The choice of antiemetic would depend upon the precise cause of the nausea in conjunction with the specific receptor affected. However, since several different neurotransmitters stimulate the CTZ, combining drugs with different mechanisms of action can often be more effective than increasing the dose of one individual drug (King and Brucker 2011). Indeed, combinations of antiemetics are often used in palliative care (NHS Scotland, n.d.). Notably, vomiting of unclear or mixed origin may respond to a phenothiazine such as prochlorperazine because, in addition to acting on dopamine and serotonin receptors in the CTZ, it also acts at the VC and vestibular area. Cyclizine and prochlorperazine are both commonly used anti-emetics in palliative care where nausea and vomiting are present in up to 70% of patients with advanced cancer (NHS Scotland, n.d.). Treating this patient population requires particular vigilance, since there may be a number of underlying reasons for and comorbidities contributing to the nausea and vomiting, and antiemetics may be inappropriate. Consideration for causes of the symptoms might include intestinal obstruction or constipation, anxiety, raised intracranial pressure (ICP), oesophageal candida, severe pain or hypercalcaemia all of which might warrant interventions other then antiemetics. Conversely, should the nausea and vomiting be identified as drug induced, then anti-emetics such as cyclyzine or prochlorperazine might be appropriate. Raised intracranial pressure stimulates vomiting centre via pressure receptors and can be problematic in patients with known or suspected brain metastases. Notable, cyclizine can be g iven to such patients, especially where corticosteroids are contraindicated (NHS Scotland, n.d.). Pharmacokinetics Cyclizine, like most antihistamines, is well absorbed from the GI tract. After oral doing the effects develop within 30 minutes, are maximal within 1-2 hours and lasts for 4-6 hours. A single oral dose of 50 mg cyclizine in healthy adult volunteers resulted in a peak plasma concentration of approximately 70 ng/mL, occurring at about two hours after drug administration. The plasma elimination half-life is approximately 20 hours.  [2]  Cyclizine is extensively metabolised in the liver via N-demethylation to the inactive metabolite norcyclizine (Figure 4), which is widely distributed throughout the tissues and has plasma half-life of approximately 20 hours. This metabolite has minor antihistaminic activity compared to parent drug. A single 50 mg dose of cyclizine when given to an adult male volunteer, results in less than 1% of the total dose administered being excreted as parent drug in the urine over a 24 h period. Thus urinary excretion of metabolite rather than parent drug is th e major route of elimination for  cyclizine. The metabolism is thought to be mediated through CYP 2D6 and therefore exhibit inter-subject variability dependent upon the CYP 2D6 genotype as demonstrated by Vella-Brincat et al. (2012) in their study of the PK of cyclizine (Appendix 1) and its major metabolite (Appendix 2) in palliative care patients receiving sub-cutaneous cyclizine. Results indicated that the metabolic ratio of parent drug to metabolite differed significantly according to CYP2D6 genetics.  [3]   Prochlorperazine is reasonably well absorbed from the GI tract and highly protein bound. It undergoes extensive metabolism both in the gastric mucosa and on first pass through the liver via the cytochrome P450 enzyme system (CYP 2D6 and CYP 3A4)  [4]  to inactive metabolites, which are subsequently excreted in the urine. Parent drug has a plasma half-life of between 4 and 8 hours, the precise half-life differing according to the mode of administration. An im injection produces its antiemetic effect in 5-10 minutes and it lasts for 3-4 hours. Onset of effects are related to the mode of administration hence the pharmacokinetic profile, thus an oral dose would have a slightly slower onset of action but would last longer compared with an im injection.  [5]  According to Finn et al (2005), although the drug has been accepted as a useful anti-emetic for over half a century, its therapeutic success has been limited by its low and variable absorption and high first-pass metabolism. H owever, the development of a new buccal formulation has improved the PK, since studies demonstrate that buccal administration of prochlorperazine produces plasma concentrations more than twice as high as an oral tablet, with less than half the variability (Finn et al., 2005)  [6]  (Figure 5). When placed in the buccal cavity between the upper lip and the gum the formulation forms a gel from which the prochlorperazine is released and absorbed. The plasma levels achieved at steady-state on a dosage regimen of one 3mg buccal tablet twice daily are similar to those observed with the standard oral dosage of one 5 mg tablet taken three times daily. The elimination half-life of prochlorperazine in this formulation is 9 hours. The safety and efficacy of this relatively new formulation has also been demonstrated by Bond  [7]  (1998) in a randomised, double-blind, double-dummy trial in patients with vestibular disorders. Side effects By virtue of their pharmacology, cyclizine and prochlorperazine are both central depressants and can cause impairment of performance (Benson, 2001). Consequently, the pharmaceutical data sheets for both drugs have warnings regarding their potential to interfere with the ability to drive or operate machinery safely due to their ability to cause drowsiness (BNF, 2012). Despite the fact that cyclizine is one of the older antihistamines it is considered less potent in this regard compared to others in its class (Broccatelli, 2010), however, there is considerable variability in response to this side effect which can range from slight drowsiness to deep sleep. For this reason in practice, when one drug is not effective or poorly tolerated then it is justifiable to give another drug or combination of drugs (Benson, 2001). This unwanted side-effect is also a feature of prochlorperazine especially in the elderly, and often diminishes with continued treatment of both drugs (emc, n.d.). Cyclizines other more common side-effects include headache and psychomotor impairment plus antimuscarinic effects, such as urinary retention, dry mouth, blurred vision, and gastrointestinal disturbances (BNF, 2012). Less common side effects are palpitations and arrhythmias, also dizziness, hypotension, muscular weakness and poor coordination (Goodman and Gilman, 1975). Prochlorperazine commonly causes CNS related side effect such as acute dystonia or dyskinesia, however these tend to be transitory (usually occur within the first 4  days of treatment) and are more common in children and young adults. Dopamine antagonists like prochlorperazine can also cause extrapyramidal effects, QT prolongation and even severe hypotension, especially in the elderly (emc, n.d.). Muscle spasms and restlessness are other reported side effects. Interactions Cyclizine exhibits pharmacological interactions with other drugs due to antagonism of its action (donepezil, galantamine, rivastigmine) or enhanced anticholinergic actions (tacrine, trimethobenzamine, triprolidine, trospium). Pharmacokinetic interactions may arise since cyclizine is an inhibitor of the hepatic CYP 2C9 isozyme system, which is involved in an NADPH-dependent electron transport pathway. This isozyme oxidizes a variety of structurally unrelated compounds, including steroids, fatty acids, and xenobiotics and contributes to the wide pharmacokinetics variability of the metabolism of drugs such as S-warfarin, diclofenac, phenytoin, tolbutamide and losartan. Pethidine and propanidid are also listed as having a potential to interact with cyclizine. Cyclizine also acts as an inhibitor of estrogen sulfotransferase, the enzyme responsible for estradiol metabolism. Prochloperazine has a plethora of interactions, both pharmacological and pharmacokinetic. The pharmacokinetic interactions are largely due to competitive metabolic interactions at the hepatic CYP 3A4 and CYP 2D6 enzymes. The CYP 3A4 isozymes are responsible for a variety of oxidation reactions e.g. caffeine 8-oxidation, omeprazole sulphoxidation, midazolam 1-hydroxylation and midazolam 4- hydroxylation, plus metabolism of structurally unrelated compounds, including steroids, fatty acids, and many other xenobiotics. Whilst the CYP 2D6 isozymes are responsible for the metabolism of many drugs and environmental chemicals, via oxidative transformation along with metabolism of drugs such as antiarrhythmics, adrenoceptor antagonists, and tricyclic antidepressants.  [9]  Consequently, the data sheet for prochlorperazine lists many drugs with interaction potential including adrenaline, amphetamine, carbamazepine, clonidine, desferrioxamine, guanethidine, levodopa, lithium, phenobarbital and propranolol. Managing Drug Therapy When managing the care of a patient, nursing staff must initially thoroughly assess the patient, then identify significant interactions between core drug knowledge (PD, PK, ADRs, interactions, contraindications) and the patients core variables (health status, age and gender, life-style and diet, environments, culture). Thereafter the nurse can plan and implement suitable interventions, which will maximise therapeutic effects whilst minimising adverse effects (Aschenbrenner and Venable, 2008). In order to achieve such objectives the nurse should ensure administration of the appropriate medication is given through a suitable route on a regular basis or as required, with ongoing patient evaluation and monitoring. Cyclizine and prochlorperazine are both considered first line treatments for nausea secondary to vertigo and motion sickness (Quigley, 2001) and are first line treatments in many hospitals in PONV (NHS, Salisbury; NHS Plymouth). A review by Matchar, et al. (2003) has suggested that oral prochloperazine may also be used as an adjunct in the treatment of nausea associated with migraine (Matchar et al, n.d.). No randomized controlled trial has been found which formally compares efficacy of cyclizine and prochlorperazine, however, two studies comparing cyclizine with perphenazine in ameliorating drug-induced emesis, have shown the former to have comparable antiemetic efficacy to this related phenothiazine drug (Dundee et al., 1975; Chestnutt and Dundee, 1986). These studies are featured in a Cochrane report (Stevenson, 2006) which investigates drugs for preventing PONV and highlights eight drugs which reduce PONV by a similar amount in this patient group, cyclizine being one. The report concluded, therefore, that the most important question to answer when treating emesis is What are the types and risks of side effects experienced by patients exposed to these antiemetics? Thus safe and effective prescribing requires the nurse to identify patient variables or comorbidities relevant to the drugs side effects, for example heart failure patients should not be prescribed cyclizine and individuals susceptible to visual disturbances should avoid prochlorperazine as per the drugs contraindications. It is noteworthy that both drugs may be prescribed in the later stages of pregnancy if considered appropriate by a doctor (Schaefer, 2007; CKS, n.d.).  [10]   The choice of antiemetic would depend upon the precise cause of the nausea in conjunction with the specific receptor affected. However, since several different neurotransmitters stimulate the CTZ, combining drugs with different mechanisms of action can often be more effective than increasing the dose of one individual drug (King and Brucker 2011). Indeed, combinations of antiemetics are often used in palliative care (NHS Scotland, n.d.). Notably, vomiting of unclear or mixed origin may respond to a phenothiazine such as prochlorperazine because, in addition to acting on dopamine and serotonin receptors in the CTZ, it also acts at the VC and vestibular area. Cyclizine and prochlorperazine are both commonly used anti-emetics in palliative care where nausea and vomiting are present in up to 70% of patients with advanced cancer (NHS Scotland, n.d.). Treating this patient population requires particular vigilance, since there may be a number of underlying reasons for and comorbidities contributing to the nausea and vomiting, and antiemetics may be inappropriate. Consideration for causes of the symptoms might include intestinal obstruction or constipation, anxiety, raised intracranial pressure (ICP), oesophageal candida, severe pain or hypercalcaemia all of which might warrant interventions other then antiemetics. Conversely, should the nausea and vomiting be identified as drug induced, then anti-emetics such as cyclyzine or prochlorperazine might be appropriate. Raised intracranial pressure stimulates vomiting centre via pressure receptors and can be problematic in patients with known or suspected brain metastases. Notable, cyclizine can be g iven to such patients, especially where corticosteroids are contraindicated (NHS Scotland, n.d.). Administration Precautions Due to its centrally acting effects, patients taking cyclizine should avoid alcohol and other depressants e.g. hypnotics or tranquillisers. Food may reduce irritation to cyclizine and since there is no interaction with food, this drug can be taken without regard to meals. The datasheet indicates it should be used with caution in hepatic disease, whilst in renal impairment there is a need for dose reduction (BNF, 2012). Cyclizine should also be used with caution in patients with severe heart failure. Other anticholinergic effects include visual disturbances, and sedation, which can make them dangerous for the elderly population or younger patients. Further, cardiovascular side effects e.g. hypotension, tachycardia, and palpitations have been reported, plus minor GI effect e.g. dry mouth and constipation. Cyclizine has a well-known abuse potential (Ruben et al. 2006). In opiate dependents receiving long-term methadone cyclizine is often taken in large doses intravenously to provide a m ore intense high. Thereafter the addict experiences depressive mood changes and a craving for cyclizine. Many individuals receiving long-term prescriptions of oral methadone have been identified as being habitual abusers of cyclizine.  [11]  Consequently, there is considerable reticence by pharmacists in prescribing the drug, and alternative treatments are generally sought. Obviously in the hospital setting there is little opportunity for such abuse, and the efficacy and cost-effectiveness of the drug would therefore take precedence over its abuse potential (Barber, 1995; Philips and Thompson, 1997). Although prochlorperazine being an antipsychotic phenothiazine drug can be employed in psychiatry, in lower doses it is usually prescribed for its anti-emetic properties. Patients taking the drug should take with a full glass of water, avoid excessive quantities of coffee or tea (containing caffeine) and also avoid alcohol. Prochlorperazine should be used with caution in patients with renal and hepatic impairment and cardiovascular disease; also in Parkinsons disease, epilepsy and in patients with a history of glaucoma. While the drug does not deliver the euphoria that is associated with many commonly abused drugs, it still has some abuse potential since it can alter mood and perception, but not to the extent of cyclizine. Moreover, dependence and tolerance can develop, which can drive the individual to continue to seek more of the drug  [12]  and result in overdose, characterised by symptoms of central nervous system depression to the point of somnolence or coma. Agitation and r estlessness may also occur in overdose. Other possible manifestations include convulsions, EKG changes and cardiac arrhythmias, fever and autonomic reactions such as hypotension, dry mouth and ileus. Managing Drug Therapy Nausea and Vertigo: In emetic patients, antiemetics should only be prescribed when the underlying cause is known, indeed antiemetic administration may be harmful when the cause can be treated, e.g. in diabetic ketoacidosis or digoxin/antiepileptic overdose. In addition to motion sickness cyclizine can be given to patients with nausea caused by mechanical bowel obstruction and raised intracranial pressure.  [13]  Once a decision has been made that antiemetic drug treatment is appropriate, the drug and the dosage form should be chosen according to the aetiology of vomiting along with core drug knowledge and patient variables. Thus prochloperazine is useful for episodes of more severe nausea and vomiting e.g. associated with diffuse neoplastic disease, radiation sickness, and the emesis caused by drugs such as opioids, general anaesthetics, and cytotoxics. Indeed, prophylactic use may be required if severe nausea is anticipated such as following chemotherapy treatment. (Aschenbrenne r and Venable, 2008). Prochorperazine may be a suitable choice because of its dosage forms, thus rectal suppositories can be useful in patients with persistent vomiting or with severe nausea and the buccal tablet dosage form is also useful in such instances. However, during use of phenothiazines it is important to monitor severe dystonic reactions, especially in children. It is recommended as a second-line treatment for vomiting in pregnancy after promethazine.  [14]   Whereas the efficacy of cyclizine in treating nausea and vomiting has already been unequivocally proven, it is only available in tablet and injectable form. Nevertheless, cyclizine may be the choice of drug over prochlorperazine in children since in this patient population the latter can only be administered orally (BNF, 2012), and therefore requires patient compliance for success. There is no evidence that either of the two drugs is superior to the other in terms of efficacy; also despite cyclizines longer plasma half-life compared with prochlorperazine, the duration of action is similar at around 4 hours. The adverse event profiles do however differ slightly, because of the differing underlying pharmacology of these two drugs. This is an important consideration in the choice of drug, alongside special precautions which, as described earlier, must be considered in conjunction with patients co-morbidities. It is also noteworthy that educating patients and their families regarding the drug of choice is important; for example warning patients against consuming alcohol with both prochlorperazine and cyclizine and warning patients against driving or operating machinery if susceptible to drowsiness with either drug. In summary, both cyclizine and prochloperazine have similar safety, tolerability and toxicity profiles despite their differing modes of action on a cellular level. Tolerability in terms of drowsiness is a potential problem for both drugs, but is generally dependent upon the individual patients susceptibility, warranting a trial and error type approach when determining which is the optimal drug of choice. Also, due to the drugs both being substrates of CYP 2D6 their phamacokinetic profiles may exhibit inter-subject variability by virtue of the different phenotypes of this enzyme which exist in the population. This differing pharmacokinetic profile would logically translate into a varied response in terms of therapeutic effects. Likewise, their potential to interact with other drugs is inextricably linked with their metabolism, namely metabolic competition at the cytochrome P450 enzyme receptor sites. Thus both drugs have the potential to interact with a wide range of other medications . Moreover, since both drugs are extensively metabolised in the liver, with excretion of metabolites in the urine, there is a need for caution in renal and hepatic disease. Cyclizine and prochlorperazine appear to be similarly efficacious with regard to their treatment of emesis caused by motion sickness. The literature is inconclusive regarding which drug would be more superior for PONV, or vertigo, and even though it has been suggested that prochlorperazine should be chosen over cyclizine when the nausea is severe, there does not seem to be any compelling evidence for this and many hospitals tend to choose cyclizine over prochlorperazine in their antiemetic protocols/guidelines. The most compelling evidence for choosing prochlorperazine over cyclizine in the primary care setting would be the high abuse potential with cyclizine. However, in the secondary care setting this is of minimal concern. Therefore a more compelling reason for choosing prochlorperazine over cyclizine in this setting might largely hinge on the greater flexibility in formulations available for prochlorperazine. Whereas both drugs can be given orally as a tablet, when patients are vomiting this may be inappropriate. The buccal tablet or rectal suppository, which is available for prochlorperazine, and is less invasive than an injection formulation may be more acceptable to many patients in such cases. To conclude, the present essay has demonstrated that the nursing process for effectively dealing with emesis is challenging and complex. Here we have witnessed the plethora of facts which the nurse must take into account prior to prescribing the antiemetic drugs cyclizine and prochlorperazine, and that even after attempting to optimise drug selection on the basis of such facts, success cannot be guaranteed. Ongoing monitoring of patient response/progress with the possibility of altering or augmenting the chosen drug therapy is necessary to improve outcomes, ensure patients receive optimal care, and that they enjoy maximal therapeutic success with minimal side effects. The Bay :: The Hudson’s Bay Company

INTRODUCTION The Hudson’s Bay Company is the oldest incorporated merchandising company in the world. The Bay has remained in business in North America after its incorporation on May 2, 1670 by the Royal Charter. The official title of the company is, â€Å"The Governor and Company of Adventurers of England trading into Hudson’s Bay†. This title is usually shortened to â€Å"Hudson’s Bay Company† or, just â€Å"the Bay. I was attracted to the Hudson’s Bay Company because firstly, I am frequently shop at the Bay. As a result of this, I would like to show the pride and respect I have for this successful company and its great history. Secondly, I have easy access to the information required to complete this independent study. Lastly, as I am not an expert when it comes to the operations of the company, this independent study will enable me to achieve a deeper understanding of management, and how it affects me. I utilized a number of resources in order to create this report. The resources are: Hudson’s Bay Company Annual Repoorts of 1996, 1997, 1998, Employee Training handbook, the Internet, and magazines. I combined these resources to produce a very informative report on the Hudson’s Bayu Company. In addition to this, the resources I used gave me a chance to cover many aspects and areas of this corporation. The topics covered in this report are: Company Profile, Human Resources, Customer Services, Organization, and Community Involvement. I included these types of information to give you, the reader, a deep interpretation of the Hudson’s Bay Company. To understand how the Hudson’s Bay Company (HBC) became the corporation it is today, a brief history is needed to gain some background knowledge. In the Bay’s first century of existence, it created forts on the Hudson Bay and traded furs to the Native Peoples. In the Bay’s second century, competition from the North West Company was acknowledged and responded to. This total time period of 200 years assisted in the discovery of Canada. In 1821, the two competitors merged under the name of Hudson’s Bay. In the twentieth century, HBC began to concentrate on retailing which is it’s primary activity today. Between 1913 and 1968, HBC made downtown department stores in the major cities of western Canada. As time rolled along, the Bay moved into eastern Canada and the suburbs of major Canadian cities.

Becoming an Effective Leader Essay

My job title is Training Co-ordinator for an organisation called Lifeskills Centres, I assess in the following areas; business administration, customer service, management, delivering level 2 and 3 and also any supporting key skills needed. I am responsible for my caseload of learners; their progress, learning and achievement of the qualification. I must cater each programme to suit the needs of the individual learner, I do this by setting each new starter off on initial assessments so that I can see what level they are currently at which will help me decide which course is the most suitable and which approach will be the most beneficial for the learner. I am also responsible for creating strong professional relationships with the learners new employers and maintaining relationships with existing employers this is for two reasons so they would recommend us to other employers so we gain a good reputation and so that they will want to use Lifeskills as a training provider for all of their staff and will not use a different organisation as it is a competitive market. I also advertise vacancies for our employers who have had good experiences in the past with apprentices and would like to recruit a new one. I agree with the employer all of the details to go on to the advert and interview all of the candidates and trial period the suitable ones for a week before sending them to the employer themselves to interview and then make the final decision. Within my role I use a number of different leadership styles in different situations and with different people in order to progress and achieve what needs to be achieved for example; I use the leadership style autocratic when I come to the end of the programme and I tell my learners what I want and what needs to be done and how it needs to be done, this is when the learners are short of time if they are leaving the organisation and don’t have much time to complete and it works the best when they want to achieve and are highly motivated. I have used the leadership style democratic when I had a group of learners in one particular placement where I was finding it hard to make them progress as they wouldn’t complete any work unless I was there  with them and they had no motivation towards the qualification. I discussed with the learners my concerns and asked how they thought we could move forward and they suggested one to one meetings, I informed the learners that this would be too time consuming and that I could afford to have a morning or afternoon out of the month for me to come in to the organisation and support them and motivate them to complete the knowledge questions, technical certificate and key skills side of the qualification. The learners confirmed that this was an effective method and I informed them that I will observe them twice a month but they do not have to complete any written tasks for me between observation as it will be done on the one morning / afternoon per month. I used this leadership style so that they felt they were included and to help me gain more respect from them and I do not think that it came across as a weakness from myself I think it showed that I was honest and realised that there was something not right and then discussed and implemented a solution. Lastly the leadership style I use not very often but I do still use is delegative which is where I let the learners make their own decisions on how they can progress through their qualification. I usually use this style when a learner has progressed from a level 2 qualification with myself on to a level 3 so they know the way the programme works and we have a comfortable relationship I also allow some level 3 learners to reference their own work in to their files. Application of emotional intelligence in shaping performance I employ the two aspects of emotional intelligence when dealing with learners and colleagues at all times as I must first understand myself and my goals, intentions, responses and behaviour and understand others and their feelings to be able to manage my learners and their progress. I empathise with learners and use the understanding of their emotions to motivate them and inspire them to complete their work. I am aware of my own feelings and emotions and control these to make sure that I do not behave in a way that is unacceptable towards learners or colleagues, I may not have to say anything to communicate how I am feeling as it can be portrayed in my body language and facial expressions which is also something that I have to control. Based on Goleman’s theory to be an effective leader I must be aware  of my own feelings and emotions and manage these to make sure that my emotions do not show in my behaviour if they are unacceptable. I must motivate myself which I do by meeting appraisal and review targets and setting myself my own goals to achieve. I must also manage other peoples emotions I do this by building a relationship with my learners and getting to know what they like and what annoys them so I can recognise this and stop it from becoming an issue where possible, it also makes the learners feel valued and that I care about the way they feel. It is also stated in Maslows Hieracy of needs that to work to your full ability and progress in your job and grow as a person you must have the following; Be in a healthy state, feel safe, feel like you belong somewhere and have some form of social life (friends, family, relationships) and have self esteem before you are able to grow personally. I must make sure that I show all learners that I care about each of them as an individual and that there progress and growth means a lot to me. I think an area that I personally have to work on is when I worry and panic about things for example we were mock inspected and I felt very anxious about it, I must make sure that this feeling does not come across to learners or employers and that the rest of my work doesn’t suffer from it. I also try to understand and manage the employers I deal with emotional intelligence as I want to build a relationship with them and trust so I make sure that I empathise with them and try not to do anything possible which may annoy them for example if I can see that an employer is very busy instead of updating them after the appointment with their learner I will ask if they would like an update via email as I can see they are really busy and then they can decide how they would like the feedback. Setting direction and gaining commitment I communicate and set direction for my learners by visiting them fortnightly, observing them work, showing them what each observation will cover or link in with, giving them feedback on their performance and setting them targets for the next appointment. I provide ongoing support via email and over the telephone to learners when I am at work but not at an appointment with them and review their progress and set targets formally at their ten week reviews which gives them clear direction and motivation to meet the targets they  agree by the next review. I provide support and tutorials to learners for the whole of the technical certificate and functional skills they complete and also offer training on the telephone, prioritising techniques and assertiveness skills as I have noticed that this is an area for development that employers ask for and the learners too. I also motivate and gain commitment from learners and try and persuade new employers to take on new learners by using myself as a role model example, as I began my career by completing an administration level 2 apprenticeship and have completed enough qualifications and worked my way up to becoming a Training Co-ordinator. I communicate and set direction and motivate employers by keeping them up to date with the learner’s progress and exactly what I have been doing with the learner after each visit where possible and including them in the qualification by asking for witness statements and also asking them to participate in the learner’s reviews. With my colleagues I communicate direction in standardisation meetings where we share best practise on how to cover and assess certain units that we may find difficult or what we have found that works very well. I try to motivate my colleagues by being hardworking and genuinely enthusiastic about my learners achieving and my job .
